Payment is expected at the time of service. Payment of fees is the patients responsibility. We participate with most major insurance providers and will assist you by billing your insurance directly for services provided. We participate with Medicare. Medicare pays a percentage of approved charges and the remainder may be covered by supplemental insurance. We recommend that you communicate with your insurance carrier to determine if your service is covered by your insurance. Your insurance carrier may require that you have a referral from your primary care physician. Please check with your carrier before your appointment. Please bring all your insurance cards with you to your appointment. We participate with most major insurances including those listed below.
